PERMISSION & RELEASE
I give permission for the child and/or children listed below to take part in services, classes, and activities of The Oil City Church, and to ride transportation provided or arranged by the church to and from those activities. This includes regular services, Vacation Bible School, youth and children's events, outings, and similar church-sponsored activities.
If a medical emergency occurs and I cannot be reached, I authorize church staff to seek emergency medical treatment for my child.
I understand participation is voluntary, and I release The Oil City Church, its staff, and its volunteers from liability for injury or loss, except in cases of gross negligence or willful misconduct.
This form stays on file and remains in effect for twelve (12) months from the date signed. I may cancel or change it at any time by telling a church leader in writing. The church will tell me in advance about any overnight trip or travel outside the local area, which requires my separate approval.
